Note that the form/s need to be completed for each individual visitor, and that the same form is used for both sponsorship and/or guarantee of accommodation (Proof of sponsorship and/or private accommodation - Bewijs van garantstelling en/of particuliere logiesverstrekking). This form also has to be notarized by a municipality.
